Anesthesia & Sedation Consent

I acknowledge that my pet is scheduled for a procedure, which may or may not include surgery with anesthesia or a sedative. I recognize and understand that occasional complications can occur due to preexisting conditions that are not evident during routine physical exams. To help minimize complications, the doctors and staff at Alma Animal Hospital have recommended that my pet be screened prior to anesthesia by means of diagnostics.

I have been informed in advanced that anesthesia/sedative will be used and I am authorizing the performance of diagnostics such as bloodwork and or x-rays. I have agreed to pay for these services.

I hereby decline the recommended pre-anesthetic diagnostic testing.

While I accept that all procedures will be performed to the best of the abilities of the staff at this hospital, I understand that no guarantee or warranty has been made regarding the outcome of the procedure. I understand that any prices quoted for such procedures are for non-complicated operations and that any unforeseen complications may result in further cost and I will be held liable for such charges.

I am aware that no anesthetic or surgical procedure is without risks, even in apparently healthy animals. I accept that unforeseen conditions may arise during the procedure and this will not relieve me from any obligation to pay all reasonable charges incurred in treating my pet.

Post Operative complications may occur such as tracheal irritation due to endotracheal tube, aspiration pneumonia, spinal injury, among other related health issues. These complications, although rare, are risks that can happen from anesthesia/sedation and I relinquish Mira Loma Animal Hospital of all liability.
